Community Benefit is central to our social mission. We believe good health is a fundamental aspiration of all people. We recognize that promotion of good health extends beyond the doctor’s office and the hospital. Like our approach to medicine, our work in the community takes a prevention-focused, evidence-based approach. To be healthy, people need access to healthy and nutritious food in their neighborhood stores, clean air, successful schools, and safe parks and playgrounds. Good health for the entire community also requires a focus on equity as well as social and economic well-being.
We focus our work on three broad areas:
- Providing access to high-quality care for low-income, underserved people
- Creating safe, healthy communities and environments where people live, work, and play
- Developing important new medical knowledge and sharing it widely with others and training a culturally competent health care workforce of the future.

Moreno Valley Medical Center Grants Program

Kaiser Permanente Moreno Valley provides grants to local nonprofit health and human service organizations that address the priority health needs for its community service area. The current funding priorities, funding timeline, and guidelines on how to apply are provided below.

Kaiser Permanente Moreno Valley is currently focused on supporting the following health needs and related strategic priorities. Reviewing this area will help to ensure that your proposed grant project is aligned with these priorities.
Access to Health Care
- Increase coverage and access to comprehensive, quality health care services for low income and uninsured populations. Support will be considered for programs that a) develop solutions that address the health care needs of people who do not qualify for low-cost or no-cost health insurance, including but not limited to undocumented individuals, and b) support the provision of high quality health care (including preventive services and specialty care) for underserved populations, c) focus on targeted outreach, enrollment, and retention efforts to increase access to health care coverage, particularly among communities of color and immigrant populations, where disparities in both access to care and health care coverage is greatest.
- Improve the capacity of health care systems to provide quality health care services, including the social and non-medical needs of their patients. Support will be considered for programs that focus on improving the quality improvement (QI) infrastructure of health care organizations by supporting capacity to use data, leadership training, business operations and through other infrastructure building strategies.
- Improve the capacity of the primary care workforce to meet community needs. Support will be provided for training for medical providers on the provision of culturally competent care to diverse populations.
- Enhance individuals' utilization of the community based health delivery system. Support will be considered for programs that reduce barriers to access to care by providing language interpretation, transportation, non-traditional access points, and/or other supportive services.
Mental and Behavioral Health
- Improve the knowledge, capacity and infrastructure of health care, organizations and communities to address mental and behavioral health. Support will be considered for programs that focus on a) integration of health care with community-based mental health services, such as: training of health care providers to identify mental and behavioral health needs, patient navigators who can help patients access services, strengthening of referral networks, and/or co-location of services between primary care and mental health providers, b) enhancing access to high quality substance abuse treatment including medication-assisted treatments to decrease the burden of addiction and promote resiliency and recovery, c) school and youth development organizations in learning about and addressing mental and behavioral health, including suicide prevention and trauma-informed care, d) organizational changes that can reduce employee stress, and e) support policies and programs that address early child development and toxic stress, including positive parenting and violence-free homes.
- Promote positive mental health by fostering community cohesion and social and emotional support. Support will be considered for programs that focus on community based initiatives that promote positive mental health by fostering community connection to one's neighbors and participation in local activities and create access to safe local public spaces where people can congregate.
Obesity/Diabetes/Healthy Eating Active Living (HEAL)
- Improve access to opportunities for physical activity in the community. Support will be considered for programs that focus on a) new and improved policies and environments that support active transportation and physical activity (e.g., safe pedestrian bicycle routes.) and b) policies that prioritize underserved neighborhoods for park investments and encourage communities to use parks to their full potential.
- Improve access to healthy food options in the community. Support will be considered for programs that focus on a) support local governments, schools and/or community based organizations to provide healthy food options (including accessible drinking water) and to adopt healthy food policies, including procurement practices, and b) support local governments, schools and/or community-based organizations to enroll community members into available food programs such as WIC, Cal Fresh.
- Improve linkages between health care services and community-level services. Support will be considered for programs that a) improve referral of patients to evidence-based health promotion programs that teach self-management and empowerment techniques for chronic disease management and prevention and b) support collaboration between health care providers and community-level services to support patients' needs related to upstream determinants of health, such as access to healthy food at local grocery stores and addressing violence-free neighborhoods.
To be eligible for a Kaiser Permanente Moreno Valley Medical Center Community Benefit grant, an applicant organization (or fiscal agent), must have operations in California and be one of the following types of nonprofit organizations:
- 501 (c)(3) tax-exempt organization with a 509 (a) designation indicating that the organization is not a private foundation
- A local, state, or federal government agency, including any of its subdivisions that perform substantial governmental functions
In addition, organizations must:
- Provide direct services to disadvantaged and/or underserved populations that address funding priorities identified in the Kaiser Permanente Riverside County Service Area Community Health Needs Assessment
- Provide services within the geographic boundaries of Kaiser Permanente Riverside County Service Area, which includes the communities of: Aguanga, Cathedral City, Corona, Desert Hot Springs, Hemet, Homeland, Indio, Jurupa Valley, Lake Elsinore, March Air Reserve Base, Mecca, Menifee, Murrieta, Norco, Neuvo, Palm Desert, Palm Springs, Perris, Riverside, Romoland, San Jacinto, Sun City, Temecula, Thermal, Wildomar, Winchester, Yucca Valley
Kaiser Permanente Moreno Valley Medical Center Community Benefit does not consider funding requests from international, social, recreational clubs, or for the following:
- Sports teams and tournaments (e.g., golf tournaments, tennis events, walks, and runs)
- Religious purposes
- Partisan political activities
- Endowments or memorials
- Re-granting purposes to other organizations
Kaiser Permanente Moreno Valley Medical Center will not consider requests from organizations that discriminate on the basis of gender, age, economic status, educational background, race, color, ancestry, national origin, sexual orientation, gender expression, gender identity, or marital status in your programs, services, policies and administration. In addition, Community Benefit grants will not be awarded for activities, events, or programs organized or solely sponsored by alcohol, tobacco, pharmaceutical companies, or companies that produce firearms.
We generally do not provide grants for academic research, capital campaigns, event sponsorships (including community health fairs), or political campaigns.
- Organizations must be in good standing with all previous grant awards and required reports
- One grant proposal per agency, per local funding cycle
- 12-month grant terms (approx. July 2017- July 2018)
- Grant awards range from $5,000 up to $25,000
- Encourage partnership grants (up to $50,000)
Kaiser Permanente Moreno Valley Medical Center highly encourages grant requests submitted by two or more nonprofit organizations partnering to address the same priority need and to provide direct complementary services to the target population. Services provided by the partnership organizations should not be duplicative or overlapping and must demonstrate a continuum of care addressing the selected priority need.
The partnership should identify a single organization to function as the Lead Agency through established Memorandum of Understanding (MOU) agreements. A completed Grant Partnership Form must be submitted with the online grant application. Note: Organizations applying as part of a Partnership Grant cannot submit a separate application as an individual organization.
- Applications must be submitted before 2:00pm on March 15, 2017
- Kaiser Permanente will announce grant awards by July 2017
