  • What are the deadlines?

    Grant Application Deadlines

    • Local: Southern California medical center service areas grant deadlines are March 15th. The San Diego service area is the exception with a grant release date of May 15, 2017 and a grant deadline of June 15, 2017. To find out more, please visit the service area website.
    • Regional: Southern California regional offices do not have a deadline. They review grant applications throughout the year.
    • National: Kaiser Permanente’s national office does not have a deadline. They review grant applications on a quarterly basis.


    Grant Final Report Deadlines (Local, Regional, and National)

    • The final report is due 30 days after the grant end date.

    Event Sponsorship Application Deadlines (Local, Regional, and National)

    • It is strongly recommended that event sponsorship applications be submitted 90 days in advance. Event sponsorship applications do not have a definitive deadline.

    Event Sponsorship Final Report Deadlines (Local, Regional, and National)

    • The final report is due 30 days after the event has concluded.
  • How can I tell if my application or report has gone through?

    When an application or a report is successfully submitted, the system generates an email confirmation from Kaiser Permanente Online Application indicating that the application has been received. This is why it is very important that we receive accurate emails on the applications.

  • If I am in the middle of completing my application, can I save it and continue working on it later?

    Yes. Please follow these instructions:

    • Select “Save & Finish Later” which can be found at the bottom of each page of the application.
    • You will be directed to the “My Account” page.
  • I selected “Save and Finish Later” and now I can’t get back to my application. What should I do?

    Go to your in-progress application, which can be found on your account page at https://www.GrantRequest.com/SID_946?SA=AM.

  • How do I submit attachments if I’m using the online system?

    Go to your in-progress application, which can be found on your account page at https://www.GrantRequest.com/SID_946?SA=AM.

    • Applicants should upload required documents using the online application system.
    • As you proceed through the application, you will be asked to upload the required documentation.
    • If you have additional questions about the attachments, please send an email using the “Contact Us” link located at the top of every page of the application, or contact your local Kaiser Permanente Community Benefit staff member.
  • Can parts of the application be filled out by my colleagues?

    Yes. Please follow these instructions:

    • Select “E-mail Draft” to email a copy of the draft application
    • Be sure to provide the account login information to the colleague who will work on the application
    • Only one person can be logged into the account at a time
  • My colleague started the application. Can I finish it?

    Yes, but once an application is started, it must be submitted under the same account (email address).

  • How do I access my account if I forgot my password?

    Go to the account login page at https://www.GrantRequest.com/SID_946?SA=AM and follow the instructions to retrieve your password

  • The person who started the application is no longer with the organization. What do I do?

    Please send an email to KP-Community@kp.org with as much of the following information as you have: Organization name, the request ID (e.g., 20612345), project title, and old and new email addresses.

  • How do I submit the application?

    1. You will be able to submit your application when you have provided all required responses and attachments
    2. Select “Review and Submit” at the bottom of the last page of the application
    3. Select “Submit” at the bottom of the page if no additional modifications are needed
    4. Once an application is submitted it cannot be modified
    5. You will receive an email confirming receipt of the application which will include an application ID #
  • How do I begin the online application process?

    Please take the eligibility quiz on our website to access an application.

  • Does Kaiser Permanente require any documentation in addition to the online grant application form?

    Yes. As you proceed through the application, you will be asked to upload the required documentation.

    Required documents for all applications

    • List of Officers and Directors and their affiliations
    • Completed budget (a template is available for download on the application)
    • W9 (a template is available for download on the application)

    Additional required documents for applicants using a fiscal agent

    • Fiscal agent memorandum of understanding

    Additional required documents for requests of $25,000 and over

    • IRS Form 990 (Not required of government entities)
    • Audited Financial Statement
    • Organization Budget

    Additional required documents for regional applications

    • Completed Work Plan (the template is available for download on the application)
  • Does Kaiser Permanente require any documentation in addition to the online event sponsorship application form?

    Yes. As you proceed through the application, you will be asked to upload the required documentation.

  • How do I access my report?

    You must log in with the same email address that was used to submit your grant application, otherwise you will not be able to access your report. Next, follow these steps:

    • Log in to your account page: https://www.grantrequest.com/sid_946?sa=am
    • Once logged in, select the word Requirements to show the Requirements page (below)
    • From the Show drop-down menu (above) select whichever of the following you would like to view (e.g., New, In Progress or Submitted) (above)
    • For new and in progress applications or reports, select the form name you would like to open.
    • For submitted applications or reports, select the form name to view an HTML version of your submitted application.
  • What do I do if have more questions?

    • If you have questions related to the content of your application, please send an email using the “ContactUs” link located at the top of each page of the application.
    • If you have technical issues, please send an email to KP-Community@kp.org with your organization’s name and describe the issue
