Community Health Needs Assessment (CHNA)

The Patient Protection and Affordable Care Act requires nonprofit hospitals to conduct Community Health Needs Assessments (CHNAs), and to make these reports accessible to the public. Kaiser Permanente is pleased to make available its 2013 CHNAs.

Conducting these assessments and making them available to the public meets our obligations under these new federal requirements. More than that, these assessments reflect Kaiser Permanente’s long-standing commitment to working with people in our communities to identify and address the most urgent health needs in the communities we serve.

The links below provide access to the CHNAs for each of our licensed hospitals. We welcome feedback on these assessments. Links to provide feedback are available on the cover page of each assessment.

For more information on the process Kaiser Permanente used to conduct our Community Health Needs Assessments and to access a free, web-based CHNA Data Platform we developed that puts comprehensive community health data at your fingertips, please visit chna.org/kp.
